MEMORY CARE

GEMMS By Liberty

Genuine, Engaging & Meaningful Moments

Liberty Senior Living recognizes that each individual is unique, in both mind and body, and for that, we want to customize the experience of each individual that we serve.

Providing memory care that supports residents with compassion, structure and engaging daily experiences. We know that memory loss affects both the individual and their loved ones, which is why our program emphasizes dignity and connection at every step.

GEMMS Daily Programming
Requirements:

  • Person-centered: Activities should be tailored to residents’ past interests, current abilities, and life stories.
  • Consistent routine: Predictable schedules reduce anxiety and confusion.
  • Purposeful engagement: Activities should feel meaningful (not “childlike”), giving residents a role or reason to participate.
  • Choice and flexibility: Offer options and allow residents to engage at their comfort level.
  • Social and relational: Engagement is more successful when built on trust, companionship, and empathy.
  • Short and frequent: Activities should be broken into manageable, shorter sessions with built-in rest.

Liberty GEMMS Pillars of Care

Abilities Matter!

Focus on what can be done, not what can’t be done.

Recognize and celebrate remaining strengths. Encourage independence in daily tasks, even if support is needed, to promote dignity and self-worth.

Behavior is Communication

Every action tells us something.

Understand that expressions, gestures, or changes in mood are often attempts to communicate needs, feelings, or discomfort when words may no longer come easily.

Connection Before Task

Build trust before moving into care activities.

Take time to approach gently, use eye contact, and connect emotionally before assisting with personal care. This reduces anxiety and fosters cooperation.

Do with, not to”

Care is a partnership.

Engage the person as an active participant in their care. Involve them in choices, movements, and routines so that care feels collaborative, not imposed.
